After you meet Opher for the first time, the NPC that upgrades your abilities, he gives you a rumor of a nearby Shrine. Head East to the Combat Shrine and complete the combat challenge there. You will receive your first Shard Slot Upgrade for completing the challenge successfully.
When you visit the Wellspring Glades you’re given a mission to reach The Wellspring. On the way there, you can find this combat shrine. However, you will need to enter Wellspring Glades first so you can unlock grapple. Complete the Combat Shrine for the upgrade.
The next one I found was in Mouldwood Depths. This is a tough area, pick up the Flash Ability before you do any serious exploring. As before, complete the Combat Shrine challenge to unlock another Shard Slot Upgrade.
